Chariton Nursing and Rehab Center is certified by Medicare and Medicaid and licensed by the State of Iowa to provide for the long term and rehabilitative need our community and surrounding area.

Our pleasant home-like health care facility is staffed by caring and professionally trained personnel from our local community. At Chariton Nursing and Rehab Center, we are committed to providing personalized service, stressing the individual’s right to dignity and respect. The entire staff is deeply dedicated to the highest standards of a team-oriented care program while providing compassion and understanding for our residents and their families.
We encourage family involvement and participation in our programs and rehabilitation process. Through our therapy programs, we strive to return individuals to their greatest degree of independence by maximizing their abilities and minimizing their disabilities.
We realize that a resident’s happiness and well-being are just as important as their health care. Our varied and well-rounded activity program includes therapeutic activities, special events, a wide variety of religious programs, as well as entertainment programs both in our facility and out in the community. Residents are encouraged to attend community functions outside the facility in addition to facility functions.
Directed by our dedicated staff, our residents have ample opportunity to be involved in both large and small group activities. Individual programming is provided to those who cannot or choose not to participate in group activities.
With the support of our many community volunteers, we feel our program offers a wide range of opportunities for involvement. We invite the community, family members and friends to join us in our fun projects.
Our qualified dietary staff works closely with our nursing department, our licensed dietitian and each resident’s physician to provide attractive, delicious, and nutritious meals. Each resident is visited by the dietary supervisor or dietitian to obtain individual likes, dislikes and special requests.
Meals are served in our attractive dining rooms providing residents opportunity to socialize during meals.
We welcome family members and friends to dine with our residents and experience our home-like atmosphere and fine food.
Our services are continually expanding in scope and programming to meet the growing needs of our residents and community. Other services include:
· Pharmacy services – delivered directly to our facility on a 24/7 basis.
· Beauty and Barber Shop services
· Social Services
· Out-patient therapy services to the community
· Selection of Hospice services – agreements with three different Hospices
· Portable X-ray services
· Cable TV
· Full housekeeping and laundry services
Chariton Nursing and Rehab is located just north of the Lucas County Health Care Center and Chariton Family Medical Center. This close proximity to physicians, labs and hospital helps ensure the continuity of care for our residents.
